Transcript Requests

Please fill out the Transcript Release Form and return to Melodie Jantzer via email or by fax at 541-474-5717.

The cost is $5 for 3 copies.  There is no cost for current GPHS students requesting transcripts.

Please allow 2 working days for transcripts to be completed and mailed, or picked up in our counseling office.
